Redefining Skin Health Through the Skin–Gut–Brain Connection

For years, people have searched for healthier skin in bottles, treatments, and cosmetic solutions. They tried stronger products. Changed routines. Covered symptoms. And blamed their skin when nothing worked.

But what if the answer was never just about the skin?

This question became the foundation of Mahmud Al-Smadi’s work and it started not in a lab or a boardroom, but at home.

It started personally

Mahmud was living under intense stress, struggling with persistent gut issues that followed him everywhere. At the same time, his partner and co-founder Annie Jaffrey was living with severe acne. Her doctors had prescribed the full conventional protocol Accutane, antibiotics, every standard treatment. It promised relief. It never solved the bigger picture.

Two people. Two different symptoms. The same root pattern.

“The skin is not separate from the rest of the body,” Mahmud says. “It is connected to what is happening in the gut, the brain, the immune system, and everyday life. When I started paying attention to that, I saw the same pattern in thousands of people.”

That observation became the foundation of Nourished3, the holistic skin health brand he and Annie co-founded in 2020. Today it operates across Los Angeles and Munich and has helped more than 50,000 people take a different approach to their skin health.

Looking beyond symptoms

What separates Mahmud’s work from conventional skincare is a shift in the question being asked. The industry has long focused on what to apply to the skin. He became interested in why the skin behaves the way it does in the first place.

The Skin–Gut–Brain Axis the connection between the skin, digestive system, nervous system, microbiome, and stress response sits at the centre of his methodology, which he calls Triple Biotic Skin–Gut–Brain. The approach draws from psychodermatology, neurogastroenterology, microbiome science, and nutritional dermatology.

The principle is straightforward even if the science is not: stress influences digestion. Digestive imbalance affects inflammation. Inflammation shows up through the skin. For many people, the frustration is not just having skin concerns it is not understanding why those concerns keep returning.

Mahmud’s work is designed to answer that question.

Bridging science and everyday life

In 2026 he published The Skin–Gut–Brain Reset, making the methodology accessible beyond the Nourished3 community. The book was scientifically reviewed by Dr. Amine Zorgani, PhD microbiologist, member of the Editorial Board of Frontiers, and advisor to the Human Microbiome Action programme bringing independent scientific rigour to the framework.

Translating complex research into practical guidance has been central to how Mahmud works. His focus is not on overwhelming people with science but on helping them understand what their skin may be signalling and what internal systems might need support.

What comes next

Building a brand and writing a book were steps. The next is measurement.

At the end of 2026, Mahmud and his team are launching a clinical study to validate the Triple Biotic Skin–Gut–Brain methodology subjecting the framework to independent scientific scrutiny before the work scales further.
